3) It isn't executed by the kernel. Firmware is not the same thing as a driver. Firmware is the software executed by an external device. For example, a Video Card or a NIC.

Take a look at this early SCSI/Floppy controller, an Adaptec AHA-1542B. You will notice that it has an Intel 8085 CPU on it. There are two ROM chips. The one on the right is the Disk BIOS that implements INT 13H and the one on the left is the Firmware for the 8085. It is labeled "MCODE FC8A." MCODE (short for Microcode) is a common (mis)nomer for firmware (it typically means a ROM used to implement a state machine or a decoder, like it does on CISC CPUs) The GM76C28FW chip in the corner is 2KiB of SRAM for the 8085. The 8085 is a coprocessor that communicates with the host CPU and the SCSI hardware. All of these chips are now typically bundled into one ASIC that sits on an expansion card. But manufacturers are cheap and want to keep the BoM as low as possible so they want to use the DRAM they put on the chip as ROM. So they have a tiny ROM who's sole purpose is to load up firmware sent by the host CPU. This ROM is very small which saves money in production. It also means that the firmware is hotfixable unlike a Mask ROM. The other way of having hotfixable firmware is by putting a Flash chip on the board. Which could add $0.17-$2.00 to the BoM. So the reason binary blobs exist is because manufacturers are so cheap that they would rather use DRAM as program memory than spend another dollar to put a Flash chip on board.
